Making an Appointment
Receiving care at Katy Trail Community Health
requires an appointment.
Appointments can be made by calling the clinic
at Sedalia 660-826-4774 or Warsaw 660-438-2717. Same day appointments are available.
Information required when making an appointment includes:
•Name, address, phone number
•Date of birth
•Reason for the appointment
•Whether you will require financial assistance
To complete the required paperwork, please arrive 15 minutes early on your first visit.
On the day of your appointment, please bring along:
• Proper identification, such as a valid driver’s license
• Your medical records, if this is your first visit
• Children’s immunization records
• Existing prescription or over-the-counter medications you are currently taking
• Health insurance card or information
• Financial Records (see Financial Assistance)
