insurance. To apply for the sliding fee, complete
an application and provide us with proof of income.
We can apply the Sliding Fee Discount only to services given by providers of Katy Trail Community Health. We cannot discount charges for the hospital, ambulance services, or outside physicians to whom you have been referred.
