Jean Moore, FNP comes to Katy Trail Community Health from another Community Health Center in Ava, Missouri and is the primary care provider at the Katy Trail Community Health Warsaw site. 

As a nurse practitioner, Moore provides primary health care to Warsaw clinic patients and information to help them make informed decisions about their care and lifestyle choices that affect health. Nurse practitioners may perform physical exams, provide immunizations, manage high blood pressure, diabetes and other chronic health problems. Moore also will be involved in patient and community education and outreach efforts.
